The BIGEPI project derives from the previous INAIL-BEEP project (“Use of Big data in occupational and environmental epidemiology”).

The BEEP project started on 29/6/2017 and ended on 28/12/2019.

The general aim of BEEP was to estimate, through methodologies for the use of BIGDATA, the effects of air pollution, noise and weather-climatic variables on the health of the Italian population.

A special focus was dedicated to the analysis of hospitalization and mortality risks in the national context and in the main Italian metropolitan areas, to the risks of accidents in populations of workers in relation to environmental factors, and to the risks of commuting accidents in relation to mobility of the population and extreme climatic events.

The project was divided into specific objectives (OS) dedicated to different spatial domains, from the entire national territory to the urban micro-scale.

OS1: to evaluate the exposure of the Italian population to various environmental pressure factors (air pollutants, extreme temperatures) and the effects on health, in terms of risk of hospitalization or occupational injury, on a national scale.

OS2: to evaluate the risk of natural and cardio-respiratory mortality resulting from environmental exposure (air pollutants and extreme temperatures) for 5 Italian regions.

OS3: to evaluate the health risk, in terms of cause-specific mortality in the general population and the risk of occupational injury in workers resulting from environmental exposure (air pollutants and extreme temperatures) for 6 metropolitan areas.

OS4: to evaluate the health risk, in terms of mortality and incidence of specific pathologies in the general population, or occupational injury in workers, resulting from environmental exposures (air pollutants and meteorological variables) defined at very high spatial resolution within the metropolitan area of Rome and the city of Pisa.

Furthermore, periodic newsletters will be produced to describe the progress of the project and to update on the reached milestones.

Although epidemiological research has strongly focused in recent years on the health effects of air pollution and extreme temperatures, further investigations are needed to estimate acute effects due to short-term exposure and chronic effects for long-term exposure in national contexts characterized by a large matrix of variability in the urban and territorial fabric. Today this necessity can be satisfied thanks to the availability of "Big Data" relating to environmental and health data that provide total coverage on the national territory with an excellent level of spatial and temporal resolution.

The main objective of the BIGEPI project is to identify the risks associated with short and long-term exposure to air pollution and air temperature in the general population, in terms of effects on mortality, hospital admissions, morbidity and physiological parameters.

The project is structured into specific objectives aimed to the evaluation of acute and chronic effects in different contexts. A special focus concerns the analysis of mortality risks in the national context and in contaminated areas such as Sites of National Interest and other areas with elevated industrial activity; the chronic effects of environmental exposure will be investigated in six longitudinal studies and in eight analytical epidemiological investigations and through a detailed analysis of the possible interaction between environmental and occupational exposure and effects on morbidity.

The results of the BIGEPI project will therefore increase knowledge on the health risks associated with environmental exposures, an extremely topical issue in view of current climate change.
